Clinical Case (Study and Observation) - Radiodermatitis Disease

 RADIODERMATITIS DISEASE
DERMATOLOGICAL WOUNDS

 

The radiodermititis disease happen with secondary complication or iatrogenic after radioterapic treatment. 

The amount and necessary it will vary according to the involved pathology, location and used protocol. Everything will be a cost subject and benefit. 

The gravity of the lesion will depend: 
- radioterapic type, 
- applied dosagem, 
- area anatomical zonet, 
- patient's nutrition. 

The cutaneous lesions can present secondary complications, as local infection.

In serious lesions the hyperbaric oxygentherapy has indication.

CLINICAL MANIFESTATION

In the topical treatment of these lesions it can be used several products, can be used: silver
sulfadiazine with cerio nitrate
, enzymatic ointment (collagenase, fibrolynosin), hydrogel, linoleic
acid
, ricinoleic a
cid, bioperoxoil, Aloe vera, elicina, papain, albumin, non adherent gauzes, calcium and
sodium alginate
ecc. 

The change period will depend on the chosen product.

CLINICAL CASE

FS, 46 years, male. Radiodermatitis in sacral area after ressection of invasive espinocelular carcinoma (Photo 1). To notice aspect necrotic tissue and fibrinous slough of the wound.

Inicial treatment in another service (surgery and radiotherapy). The ressection area had been reconstructed with mio-cutaneous rotation of remnant of gluteous massimus and the skin-graft in the donor site.

After the radiotherapy, the patient presented partial necrosis of the reconstructed areas (Photo 1). In this case the hyperbaric
oxygentherapy
can be indicated, however the patient did not congregate clinical conditions to be submitted to the related procedure. Dressings with
collagenase had been made 2 times/day, by 10 days.

With the improvement of the wound aspect, the dressing was substituted by gauzes impregnated with linoleic
acid
(essential fatty acid), changed each 12 hours, by three weeks, when the spontaneous healing occurred.

This case was been about interdisciplinary approach, in the Clinical Hospital of the State University of Campinas (San Paulo - Brazil) by Reconstructive Surgery and Microsurgery Unit - Departament of Orthopedia and Traumatology (then chief L.C.Candido, MD, PhD) and Hyperbaric Medicin Unit (then chief P.E. Iazzetti, MD, PhD).
